Public Records Requests
To make a request for public records from the City of Mill Creek you are given two options. For your privacy and convenience, we request your review of the following information and choose of one of the two options described below.

Records From City Departments

If you would like to request records from any city department excluding the Police Department, please complete the City Public Records Request Form. You may submit this city request form at the City Hall Reception counter between the normal business hours of 9 a.m. to 5 p.m. The form may also be submitted via email, faxed to 425-745-9650, or mailed to:

Records From the Police Department
If you would like to request records from the Police Department, including police reports, please complete the Police Public Records Request Form. You may submit this police request form at the Police Department counter (inside City Hall) during the normal police business hours of 9 a.m. to 5 p.m.

The form may also be submitted via email, faxed to 425-745-4680 (police requests only to this fax number), or mailed to:

Additional Information
Once your request is received, the city will respond to your request within five business days as required by RCW 42.56.520. For more information regarding public records requests, email City Hall or you may also email Mill Creek Police Department.
